My completion of this registration form indicates that I agree to comply with the rules and regulations of Hamilton Hills Preschool and MDO.

A non-refundable registration and supply fee totaling $130 per student due at the end of this registration

May 2020 non-refundable tuition paid by check and mailed/dropped off at the church, due by June 1, 2019. Please make all checks payable to Hamilton Hills Preschool.

September through April tuition payments will be due on the first day of each month. A $10 late fee will be assessed for payments received AFTER the 1st of each month.

I Agree:

If my child is absent from the program for illness or any other reason, I will continue to pay tuition until he/she is formally withdrawn from the program. I will notify the Preschool Director at least 2 weeks in advance should withdrawal become necessary. 

My child shall be in good health and free of communicable diseases each time he/she participates and will have the necessary immunizations.

If it is thought that any modifications need to be made to the classroom for the child, the parent must discuss this with the Director prior to enrollment.
